The over arching goal contained in the Modernizing Michigan Medicaid Long-Term Care Task Force report emphasizes the role of consumer choice and a desire to help Michigan's long-term care system to become increasingly robust and responsive to the needs of our citizens.
Using a person-centered planning process is an effective means to move from a traditional disease focused care model to a more holistic focus. Roles, services and responsibilities are changed.
Incorporating the Definition, Core Values/Principles and Essential Elements of Person-Centered Planning Process for Long-Term Care Supports & Services, Settings and Programs, as adopted by the
Long-Term Care Supports and Services Advisory Commission and with support from the Michigan Department of Community Health leadership to facilitate the adoption not only in long-term care but across the department to ensure the same sets of expectations for all services sytems, and implementing the associated activities will result in changes that include:
support the authority of the individual,
enhance the role of natural supporters,
balance reliance on clinical professionals, and
focus the team on community-building efforts.
