Leave of Absence

 Introduction
If you will be gone more than five (5) work days using sick leave, or taking unpaid leave time, you are required to request a leave of absence.  The links under each type of leave will give you the forms and information you need.
 Medical Leave, Parental Leave, or Family Care Leave

C38, Medical Certification by Physician - Employee or Family Member
 Educational Leave, Military Leave, Waived Rights Leave, or Plan C-Unpaid Leave
